Skilled Nursing Care
Continua Home Health skilled nursing care provides those facing long-term diagnoses with a long-term plan for Continua care that eases concerns and exceeds their expectations. We also provide short-term care, assisting with recovery and rehabilitation. Everyone served by skilled nursing care is unique, and our staff respect personal history, vital connections with friends and family, skills and dignity.
There are several speech and language disorders that can be treated with speech therapy including articulation, fluency and resonance disorders. After determining your needs with an assessment, we provide speech therapy exercises for adults to help with swallowing, speech, language and cognitive communication. Exercises may involve problem solving, conversational tactics, breathing and strength exercises.
We enable you to live life to its fullest by helping you prevent – or live better with – injury, illness or disability. This starts with an individualized evaluation, during which you and your family and occupational therapist determine your goals. Then, we’ll create a customized intervention to improve your ability to perform daily activities and reach those goals. Outcome evaluations are conducted throughout the process to ensure that the goals are being met and changes are made to the intervention plan as needed.
